What is Orthodontic Treatment with Jaw Surgery?

Orthodontic treatment with jaw surgery, commonly known as "jaw surgery" (Orthognathic Surgery), is a treatment that combines orthodontics and jaw bone surgery to correct severe abnormalities in facial structure and bite problems.

"For patients with severe bite abnormalities that cannot be corrected with orthodontic treatment alone, combining orthodontics with jaw surgery is the most effective solution."

This treatment requires collaboration between an orthodontic specialist and an oral and maxillofacial surgeon.

Difference from Regular Orthodontic Treatment

Regular orthodontic treatment focuses on moving teeth into proper position but cannot correct abnormal jaw bone structures. In contrast, orthodontic treatment with jaw surgery can correct both tooth position and jaw bone structure.

Reasons for Combining Orthodontics with Jaw Surgery

  • Jaw Bone Abnormalities
    • Protruding/receding chin (mandibular prognathism/retrognathism)
    • Maxillary protrusion causing protruding upper lips
    • Open bite, deep bite, or facial asymmetry
  • Breathing Issues
    • Narrow airways causing breathing difficulties
    • Sleep apnea and loud snoring
  • Functional Problems
    • Speech difficulties and inefficient chewing
    • Abnormal tooth wear from improper bite
  • Quality of Life Impact
    • Reduced social confidence and daily life challenges
    • Teasing due to abnormal facial features
  • Orthodontic Limitations
    • Cases beyond tooth movement correction
    • Inability to correct bone abnormalities without surgery

Examples of Bite Problems Requiring Surgery

Protruding Chin or Mandibular Prognathism

Characteristics: Protruding chin, lower front teeth ahead of upper teeth, elongated lower face

Correction: Cutting the lower jaw bone and moving it backward. In some cases, the upper jaw may also need to be moved forward.

Short Chin or Mandibular Retrognathism

Characteristics: Protruding upper front teeth, full upper lips, convex facial profile

Correction: Moving the lower jaw forward to create balance in facial structure.

Open Bite

Characteristics: Gap between upper and lower front teeth when biting

Correction: Orthodontic treatment combined with jaw surgery provides more stable results than orthodontic treatment alone.

Facial Asymmetry

Characteristics: Uneven left and right sides of the face, crooked chin, abnormal bite

Correction: Using 3D technology to plan surgery for adjusting bones on both sides to create symmetry.

Gummy Smile

Characteristics: Excessive gum showing when smiling (more than 3-4 mm)

Correction: Cutting the upper jaw bone and moving it upward (Le Fort I impaction).

Orthodontic Treatment and Surgery Steps at CDC Dental

Step 1: Initial Consultation with Orthodontist

  • Comprehensive evaluation and records
  • X-ray, photo, and dental scan bundle is available for 3,000 baht

Step 2: Surgical Consultation

  • Our orthodontist will refer you to a board certified oral and maxillofacial surgeon
  • Our staff will provide directions and help coordinate your appointment

Step 3: Advanced Imaging

  • CBCT scan at hospital (approximately 4,500 THB)
  • Some cases may require bone scan to assess jaw bone resorption
    • Bone scan is important for patients with abnormal jaw bone resorption (condylar resorption) because operating while the bone is still resorbing may lead to unstable treatment results

Step 4: Begin Orthodontic Treatment

  • Installation of braces
  • Pre-surgical alignment of teeth (usually 12-18 months)
  • During pre-surgical orthodontic treatment, teeth may appear worse than before because the orthodontist aligns teeth with their true bone structure, not the current abnormal structure. This differs from regular orthodontic treatment which tries to make teeth look better from the beginning.

Step 5: Jaw Surgery

  • Performed by board certified oral and maxillofacial surgeons at Dental Hospital Mahidol University or Masterpiece Hospital
  • Hospital stay of 1-3 nights

Step 6: Post-Surgical Orthodontics

  • Continues approximately 6-12 months after surgery
  • Fine-tuning of bite and alignment

Step 7: Retainer Phase

  • Long-term retention to maintain results
  • Regular follow-up appointments

Surgery First Approach

CDC Dental also offers the innovative "Surgery First Approach" for suitable candidates. This technique involves performing jaw surgery before or shortly after beginning orthodontic treatment, allowing patients to see facial changes immediately without waiting for pre-surgical orthodontics. Benefits include faster results, shorter overall treatment time, and reduced psychological impact. However, this approach requires complex planning and isn't suitable for all cases.
